Case Management is the coordination of home and community services to ensure our clients care and support needs are met on an ongoing basis. Specialised staff assess each client's needs and tailor a plan to suit our client's care plans.
Our Case Managers are skilled professionals in aged care who embrace the principles of advocacy and person-centred care planning, ensuring that each client has a comprehensive suite of services that meets their needs. Case Management is available through funded Home Care Packages or can be purchased through our Private Services.
Purchasing case management on a fee for service basis is available and does not require an Aaged Care Assessment Services (ACAS) assessment. Please contact us to discuss this option and how it may suit your individual requirements.
