Case Management & Community Aged Care Packages (CACPs)
Case management is a specialised service offered to clients who have complex needs in the community. Our case Managers are skilled professionals who embrace principles of advocacy, assessment, care planning and care coordination ensuring that each client has a comprehensive suite of services to meet their needs.
Services are delivered as part of a Community Aged Care Package (CACP), a package of care that is individualised to meet your needs. CACPs aim to provide a suite of services to clients with complex, low care needs and may include:
- assistance with showering and dressing
- washing and laundry
- arranging social activities
- transport
- medication management and
- a range of other services required to help you live safely and well at home
