C.A.R.E Program
Convalescence and Recuperative Enhancement Care
BlueCross Community and Residential Services and HBA have joined forces to offer HBA Health Insurance members the C.A.R.E Program.
C.A.R.E was developed in answer to HBA members' specific needs and is available to HBA members who find it difficult to return home immediately after a period of hospitalisation.
The aim of C.A.R.E is to provide recuperative care in a comfortable environment to achieve optimum levels of health, mobility and independence in preparation for returning home.
The C.A.R.E Program Provides:
- Private bedroom with ensuite bathroom
- Meals, laundry and servicing of room
- Emergency call system
- 24 hour supervised care
- Assistance with daily living requirements
- Coaching for increased independence
- Provision of ongoing treatments
- Access to allied health services
- Recreational activities
- Monitoring of continuing recovery
Referrals accepted from:
- Social Workers
- Case Managers
- Discharge Co-ordinators
When an HBA Health Insurance member is referred to the C.A.R.E Program the care requirements are established and these are carefully matched to the most suitable residence for the member to access convalescent care.
Post discharge services are organised by the hospital before the member's discharge.
Convalescence is provided for 7 days (longer stays will only be considered for approval in special medical circumstances).
There are some requirements to fulfil to be part of the C.A.R.E Program. Please talk to the nursing staff or social work department at the hospital for more information about C.A.R.E.
