BlueCross has a dedicated client relationships team who work closely with acute hospitals, general practitioners, case managers and other external agencies. Person centred care

Person centred care is about a special and unique story, each client’s journey, past and present and the part we now play. By knowing what matters to the person, what makes them laugh and what makes them cry, where they have lived, worked, what they have done and what they still want to do!

It is about respecting and supporting choices whilst keeping the person comfortable and safe. It is about working with the client and the family, listening, learning and sharing about what is really important and what will ‘make their day’.
